180 clinical and social data elements essential to dyad care were catalogued through expert review and survey.

Manual expert review combined with Python-assisted lookups aligned each dyad element to standard vocabularies.

Mappings were reviewed with neonatologists, addiction specialists, and informaticians for semantic accuracy.

Coverage was quantified across USCDI v4 and FHIR R4, showing 91%+ of dyad data needs already supported.

Wireframes and high-fidelity prototypes were iterated in Balsamiq and Figma based on clinician workflows.

Realistic dyad datasets were generated with GPT-4 to populate the prototype without exposing real patient data.

A SMART-on-FHIR front end was built, querying medications, labs, vitals, and notes for linked mother-infant pairs.

Usability walkthroughs with clinicians refined layout and alert logic; HHS and hospital leadership endorsed pilot advancement.

Outcome

Published in JAMIA (2025); prototype endorsed by HHS stakeholders for a hospital pilot and cited in ONC interoperability guidance.
